Add sections, markup fixes, nits

In prep for new CR-REC, I've cleaned up the spec a little bit (all editorial/chore changes):

• adds sections for interface attributes, methods, and dictionary members
• adds an index with all the defined and dependent terms
• cleaned up some of the markup, specially the
• removed some redundant things
