FHIR build context generator #82

We need to add a json context generator to the FHIR build stack. We can use one of the existing generators as a template -- see: git/HL7/fhir/tools/java/org.hl7.fhir.tools.core/src/org/hl7/fhir/definitions/generators/specification for examples

@balhoff are JSON-LD context files generated in the build process now?

Discussed on today's call. Not essential as part of the spec. Would be a good side project.
