I tried doing this initially, but GH didn't like it, so i added the short message. Maybe you'll have more success than i did?

hrm,, I've set default branch of my fork with a source of this PR, and it seems five templates are working... https://github.com/himorin/i18n-request/issues/new/choose

I'm not sure what kind of automated generation script would run behind GitHub UI, but one possibility is this file was kept (not deleted completely) as blank file, at that commit, but not deleted as this PR...? https://github.com/w3c/i18n-request/tree/0191ef96313e38f223a38095f3094f8dcb6251a9/.github/ISSUE_TEMPLATE
