Define terms locally to reduce navigation out of page #76

Closes #47 as discussed. Opening as a PR for ease of review.

This makes defined terms all link to a term definition within the specification, some of which are signalled as being defined by other specifications, with links out from the definition only. This achieves the goal of not surprising readers clicking on terms by taking them to a separate specification.

However, I think it'd be reasonable to argue that this change makes the specification worse in the sense that the index of terms in appendix A now shows the same terms defined both internally in the document and externally in other specifications, which is potentially confusing.

hrm,,, I've checked around for a while after past discussion, but I could not find any other (or better) way on this point... Checked issues at respec but found no related ones.
