Update permissions algorithms to respect existing microphone permission exception #130

Some of the prose moved from permissions spec in https://github.com/w3c/mediacapture-output/pull/128 isn't fully compatible with the microphone exception this spec mandates, so we should make sure to update it so the microphone exception is preserved.
