Add examples using EnvelopedVerifiablePresentation #287

Fix #233

Note, I have disabled the plugin that generates signed examples because it does not yet support EnvelopedVerifiablePresentations.

I'd like this to go in first, then will open another PR when https://github.com/w3c/respec-vc/issues/39 is completed.

PR for fixing rendering is here https://github.com/w3c/respec-vc/pull/40.

I'll leave this open for 1-2 days to see if we can do it in one fell swoop.
