fix Notes and Examples visual weight and flow #40

Previous designs emphasized the notes and examples. Is that what's wanted? It draws readers' attention to that text instead of the main text. It breaks up the flow — especially the more recent version with lots of padding.

Perhaps better to use simple inline text — no heading, no shading, no borders, no margins or padding?

Note: Not defined in all languages. Example: ESP is an initialism for extrasensory perception.

Or maybe the notes need to stand out a bit, but not examples?
