fix Notes and Examples visual weight and visual flow #2873

Previous designs emphasized the notes and examples. Is that what's wanted? It draws readers' attention to that text instead of the main text. And it breaks up the visual flow — especially the more recent version with lots of padding.

Perhaps better to use simple inline text — no heading, no shading, no borders, no margins or padding?

Note: Not defined in all languages. Example: ESP is an initialism for extrasensory perception.

Or maybe the notes need to stand out a bit, but not examples?
