Rename "open questions to considerations". Add some additional considerations. #141

Renames "Open questions" section to "Considerations". Most of the sections weren't really "open" questions anyway, but describing limitations (eg feature detection), that we aren't really proposing to solve.

Adds new considerations for:

Cross-origin fullscreen popups
HTTP Redirects
Fullscreen hidden content.
