Add Change Log for Third (and Second) Release

For the third release:

Remove empty Summary section
Add change log section describing changes between first and second and second and third releases
Add id's where necessary to allow internal hyperlinks to work
Update explicit URL-based references to WoT documents with ReSpec references.
