Modified metadata proposal with flat generic schemes #86

A number of modifications to the initial proposal including:

simplified flat naming of security schemes
use of "generic" security schemes that only resolve to specific mechanisms when combined with a concrete protocol
incorporation of resolutions of issues around use of OCF scheme, proxies, and OpenIDConnect

Updated to include OAuth example and flow parameters. To do: reconcile example with latest TD spec; I suspect the examples are not conformant, for instance, the use of "method".
