linking issues raised in #191 #340

This PR changes the recommendation to include a link in all publication resources to such links being optional. It also reinstates that the primary entry page must link to the manifest in all cases, even when embedded.

Opening this for further review and modification, as I think we all agree on what we want to say but aren't all sure we're saying it yet.
