If future registrations are ignored, how are upgrades or portable apps that may change locations handled?

@cryolithic: upgrades are handled by embedding the app's version number in the sentinel filename, so version bumps will cause registration to happen again. I don't think that location changes are handled (except if the version number then changes in that new location).
