In index.php the following debugging errors are shown:

Coding problem: $PAGE->context was not set. You may have forgotten to call require_login() or $PAGE->set_context(). The page may not display correctly as a result
This page did not call $PAGE->set_url(...). Using http://localhost/m/plugin-review/admin/tool/resetpasswords/index.php

The page already has a require_login() function. using $PAGE->set_url function affects the process when CSV file is uploaded
