add leaflet map generation script #3

This adds a node script that creates GeoJSON to make a choropleth map, plus a map html page.

The generated GeoJSON file is 7MB, so I didn't include it in the commit.

You might create a gh-pages branch and put the generated GeoJSON and map on that branch so that github will serve it nicely.
