Add extra old testament columns to the model #83

In the csv I added the columns "title_ot_nl | title_ot_en | title_ot_code". We need to add these to the model. The difficulty is that there can be multiple old testament commandments, relating to the same commandment of Jesus. By example commandment nr 21 in the csv.

I thought of a new way, to inlcude the OT laws together with the bilbe references. This makes it more easier, but also with a better overview.
