Rendering of QR code fails when creating MFA device #254

When creating an mfa device (awless create mfadevice) the following error appears:

[info]    KO create mfadevice
          scale qrcode: can not scale barcode to an image smaller than 41x41

Previously, a QR code was rendered in the terminal which could then be scanned in by a QR code reader or authenticator app.
