pdf2djvu should support JB2 shape dictionaries shared accross pages, at
least for bitonal images.

But I thought that was a limitation of djvulibre, so unless djvulibre supports 
shared dictionaries, pdf2djvu can't, right?

No; pdf2djvu is already working around some limitations of DjVuLibre.
Tools to create DjVu documents with shared dictionaries are readily available: 
http://minidjvu.sourceforge.net/
