2 years ago I was wrong, it happens when switching from any tool. If the menu says Zoom In but you really have the Rotate tool selected, then try to select the Rotate tool again, it switches the menu to Rotate, but nothing else changes.
This only seems to happen when switching from the Pen to the Rotate tool, and both tool and cursor are correct, just the menu item is wrong. I would blame the state -> ui translation, but it doesn't appear to happen when switching to the Rotate tool from other tools (may need to test that further).
