Adding and improving name capitalization rules

One more set of changes I've been sitting on. It allows the Mc and Mac rules to be found throughout a name, excludes some cases in which lower case is appropriate, adds more prepositions and conjunctions that are customarily lowercased, and checks that the last word of each name is title-cased in major English-speaking countries.
