This happens when the menu window is not in focus. This might happen when Settings is also open and that is the focused window of the app. But even then (at least on Sequoia) I have the first click respond properly (brings the menu into focus and the item clicked on interacts properly).

Can you add a bit more context on how/when this happens + on what macOS version?

Oh yes, it only occurs when the Setting window is also open! Good shout.

I see. Well, in that case this is considered normal. If the app has two windows and currently the Settings window is the key window, you need to click on the menu to make it key window and then you can interact with it (but still, it seems to be happening in one go for me on Sequia, tried now on two Macs).
