sample JSON for dynamic demo #12

Hi,

In the panel html demo, there was a sample csv file provided so its easy to 
follow along the samples for further customization.

However, in the dynamic html demo, there is no sample JSON format provided so 
I'm having a hard time following the examples.

I know a typical JSON format but I don't know it is formatted in 
https://storelocator-go-demo.appspot.com/query?callback=? so the provided 
medicare-dynamic-ds.js file can parse it properly.

So can you please provide/attach the sample JSON file that was used to parse 
the data in medicare-dynamic-ds.js so I can customize it too?
