Changes to the grid, account pages account navigation, wish list, side bar etc. #2

I've removed some default CSS classes that are not used - hoping that this isn't confusing, I also changed some of the grid around (so the page isn't a full 'container', the footer has its own container, middle content has its own container and sub-header does too (to allow people to have full width content if they wish to).

Also started on the top bar, simply made it a navigation for the time being, will probably work on a module to get the classes right for active tabs.

Maybe are you looking at something like https://github.com/webgriffe/module-bootstrap-menu?

Nice! I didn't see that, made something for the time being, but yours is a lot better than mine. Will use that! Thanks.
