index serve: serve default html page (for RWP) instead of accepting generic html #147

instead of accepting arbitrary html which can pose a security risk allow customizing via indexAppTag and indexScript additional query args, which default to 'replay-app-main' and './ui.js' respectively, and serve html if 'indexScript' is set
