Change Subject and People search to use AND instead of OR

This would mean that searching for "Henry Wellcome" doesn't return other Henries.

Where there's only one result, it redirects to the page, this means that the following now redirects to the Henry Wellcome page:

http://alpha.wellcomelibrary.org/people_lookup?name=Henry+Wellcome
