Library membership - form fields #7648

cc @cbowskill Summary of my chat with David L and Edward about the form fields...

Current paper form fields:

This form will need to be updated to align with the digital form:

Staff Use Only Area

Pre-reg form fields (from Wellcome library site):

Proposed changes to digital form fields

Title - Remove as this isn't used on library cards. Consider asking for preferred pronouns instead.

Permanent home address - This needs to be kept as part of agreements for access to items in the collection and e-resources. Also used for security purposes to check that people are who they say they are. Needs to include the postcode field too.

Username - Remove as no longer needed.

University/College/Institution affiliation - This field can be dropped as it has no use after the data is inputted into Sierra. Could possibly be added as a free text field under the Student options within "Which of these best describes you?"

Mandatory fields

These fields should be kept for now too, but Amelia has done some work to help reconsider these questions (which David L will be providing):

I've uploaded the updated online registration form designs to Zeplin. All the form fields featured are the ones that were must-haves as outlined above: https://zpl.io/aNWemx9

I created two versions of the address form fields, but the final approach will need to be determined by the dev implementation that will work best:

The main updates are:

@cbowskill with the considerations for the postcode field, I think this will be dependent on the dev implementation?

I've also created a secondary approach for the flow with the info loading on the same page section by section (a bit like they do on popular shopping carts at the moment) and with the last page being where they can review their details before confirming: Screen 1: https://zpl.io/RMgZqPK Screen 2: https://zpl.io/AmkrvEP Screen 3: https://zpl.io/z854Mnz Screen 4: https://zpl.io/g8qwnNM

In terms of going backwards and forward, in the first design approach, I would expect the three checkmark headings at the top to be clickable, and for the information they've entered previously to remain. On the second design approach, they would click "change" to be able to edit the info entered.
