You have a lot of "Access denied" to a lot of file accesses in the log. Do you
somehow got two PMS instances running or are you running as the wrong user?

I cleaned installed and deleted everything. The Access denied issue seems to be
gone but the streaming still doesn't work. "data is corrupt" for both local
streaming & icefilms. Here's the new debug file

Can you play any other file? SHB swapped mencoder back to an older reslese some
time ago (since the new made nothing play on my TV). Try and swap mencoder. In
the win32 dir you'll see two mencoders: mencoder.exe and mencoder_sb.exe.
Rename mencoder.exe to mencoder_old.exe and mencoder_sb to mencoder.exe and try
again.

Thanks! that fixed the local streaming but Icefilms still gives me the data is
corrupt error. Any way to test or fix that?

You're using a PS3 and you are bound to hit the folder depth limit (which is 8)
and if you do you can't browse any deeper into the tree (and IceFilms are
sometimes deeper). The FolderLimit is a workaround to this, currently due to a
bug in the PMS core made by others this doesn't work right now.
