We have multiple APM's Can't Chose Which App #11

As title says, we have multiple applications, but I can't pick one I want to use and display data. Also would be cool if we could somehow map the server ID to server Name . That way we could create like group and chose which server to display info from. OVERALL AWESOME JOB!!!

If I have some time. I was going to rework the application ID to be like the server ID drop down in the query editor. Until then, the best solution is probably just to create a new data source for each application.

Leaving a reference here just in case. This PR adds support for multiple APM applications, allowing the user to set the app_id in the metric.
