  1. add Counties table and model
  2. add country_id foreign key to States model
  3. create USA and Canada in Counties, populate States accordingly

countries table added, simple editing supported, see 9258f3fb47e71f6c74d8b8a8da6c4abaa679b738 i've done data entry for scotland and spain now see f6a1d4611e7e0210205e9697b5dc182ed8521a5e closing this
