Better clarification of commit examples with most common use case.

Oh the irony of PRs on github. I think they automagically place ellipsis in commit title. 😞 Sigh.

I feel the order is incorrect. Perhaps creating a branch, then example, then explaination?

Just a thought. Seemed like I had to read a bunch before I got to a copy & pasta example.

@zcorpan also realized we removed the "Fixes" from description. Updated to reflect issue #97. Hope that's what you expected.
