Refresh ipr-policy-update branch with LWG suggestions #149

I think something went wrong here as it includes a number of unrelated changes.

That's possible. I did merge in my branch which was based on Master as of yesterday, so it includes the unrelated changes in master as well.

The problem is likely that this target the ipr-policy-update branch but is based on a more recent master than the target branch. I'll rebase just the new changes onto the branch which will make them show up on https://github.com/whatwg/sg/pull/115 instead and then close this.

I rebased the changes onto https://github.com/whatwg/sg/pull/115 and will apply some additional fixes there.
