Quests Impossible to Complete #66

What steps will reproduce the problem?
1. Receive quest for "Kill Fighter" or "map planet"
2. Complete either quest by killing the fighter or mapping the planet
3. Return to the Station that issued the quest

What is the expected output? What do you see instead?
Most quests complete successfully but I have done both "Kill Fighter ___"
and "Map Planet _:_ Orbit _" and cannot receive a reward.

Please provide any additional information below.
I mapped the entire planet of the quest, and even placed a "improved
rovers" on the planet for a long duration, more than once, and tunneled
through mountains on my own to see if there was anything missed. Every spot
is checked out. And on the headhunter quest, I did kill the named Fighter
"Adder" I believe it was, and there was no result.
