Need to clarify ICF's EF coding conventions. #6

ICF, 2001: p. 225-226 spells out 3 coding conventions for EF. Need to clarify when EF coding is necessary and how.

After the Banff WHOFIC Network meeting, it decided to define "Environmental Factor Role" as codable entities. The relationship between "Functioning Situation" and "Environmental Factor Role" is left as what coding rules should specify.
