Create customized header for iCAT-X #18

WP header should be customizable, at least as much to give the impression that is a fully dedicated tool for editing the WHO-FIC Foundation.

At minimum, the "Home" button should be taken out (from the generic user's view, while admins might have it), so that the user cannot leave the project to browse other projects. If the user signs out, it should land on the sign in page (which should also show some iCAT-X logo).

Ideally, the WebProtégé logo should be replaced with a dedicated iCAT-X logo. Both in the header and in the browser's tab icon. An alternative solution would be to hide the WebProtégé logo from the header, and make the project name (which we can call it "iCAT-X") more prominent (bold and/or different font and/or bigger font size).

Note: While implementing this feature we have to think also about the user experience when people would edit different "versions"/"branches" of the WHO-FIC Foundation in iCAT-X.
