A generic widget should support the selection of entities from a hierarchy, starting with a configured top-level node (e.g., ICD Categories, or ICF Categories).

This generic widget should be used in the form for editing the base exclusion inclusion terms.

This requirement is similar to the editing of subclassBaseInclusionTerms described in issue #5, which seems to be already supported (as I explained in my comment). However, in this case we also need support specifying an additional name for any given base exclusion class (e.g. with an annotation on the baseExclusion assertion statement)
