Add User menu #559

Per mockups (https://wholetale.org/wholetale-css-mockup/src/settings.html), implement the "User" menu with "Logout" option.

User icon is displayed in nav bar replacing the current Logout icon
Selecting the user icon displays a menu containing the following:
- Gravatar and name
- Logout
Selecting logout logs the user out of WT
