A table would be a good format for this information, but is probably too much work. So instead, could modify AFCH to add more info to the template:

Genuinely curious, is there interest in this? I don't recall this ever being discussed at WT:AFC. Not saying I'm opposed, necessarily, but there should be a consensus to implement a change like this.
