factorize concept cooccurrence counting in a function

Create a file `commons/concept_tools.py` and create a function `count_concept_cooccurrence`.

Update the codes:

• `src/pipeline/pipeline_component/concept_relation_extraction/concept_cooc_metarelation_extraction.py`
• `src/pipeline/pipeline_component/concept_relation_hierarchy/subsumption_hierarchisation.py`
