Hey @CarrieTHA What's going on with this Jotform? The logo should be a png and the typeface is wrong. We can make these ourselves and link to a Google Doc. Or is it being zapped somewhere else? Also, why isn't it being made with Hubspot? I'm just looking around right now for where to organize new things. Will send you a little thing soon. Hiii. Best, Jeannette

Hiiii 👋🏼 LOL

Ok so here's what has happened. The Hubspot forms wouldn't let me add conditions without paying ridiculous amounts. We are trying to ask for a lot of info, so wanted to add conditions so that it would be as concise as possible. Jotform does and connects to Hubspot to put the data from the form on each of the contacts.

I created this page with an embed so that we could adjust the form but be able to keep the link we're sending out the same because we now have forms coming from multiple sources. 😵‍💫 Trying to keep it easy for us to get the right info as we've now got A LOT of people filling out the form.

I can give you access to Jotform to work on formatting. There are some options, but I tried to pick a font that was close for the moment.

@CarrieTHA Just double-checked and this looks better for now. Do we want to use the catalyst icon up top instead of repeating the logo though?
