Empty container sample application #19

As part of the Getting Started documentation, and for various smoke tests, it 
would be helpful to 
have an "empty" GuiceBox application that simply starts up and does nothing, 
but can be used for 
smoke-testing failover, JMX, etc.

It would also be useful as a starting point for users to develop new 
GuiceBox-based applications, so 
the source code should include "put your XYZ code here" comments.

Created sample.empty.EmptyStandalone and sample.empty.EmptyFailover which have 
been useful for smoke 
testing various failover scenarios and fixing some shutdown bugs.

Leaving this open to test JMX functionality. Once that's done, it should be 
handed over to documentation.
