skipped or cancelled dates should not appear in the calendar / survey #17

The auto-allocation algorithm properly handles skipped dates. However, the survey does not suppress them. This isn't a big deal, but it does have implications when a worker enters limited availability... they could enter just the right number of meals, and then the cancelled meal is removed and they suddenly don't have enough availability to fulfill their work obligations.

This actually might not be that big a deal... since the low availability worker could be relieved a meal if we're cancelling a meal.
