Fix #121 - weird menu builder behavior #191

This inlines the menu-builder from https://github.com/wilzbach/menu-builder.

In general I hope that we can replace the menu (which was only intended as showcase) with integrated controls (like the zooming control). See https://github.com/wilzbach/msa/issues/149 for more details & infos.

@guidorice, @smoretti I will just go ahead and merge this, s.t. you can give it a try with latest, which you can take directly from the S3 bucket to avoid caching from Amazon CloudFront.

For me the latest version (downloaded 2016-08-05) fixed the problem Thanks
