Request to change search for medication names #15

Initial search on medication name no longer required -- instead make available the following search criteria:

Medication name; freetext, options: common name, brand name, generic name)
Administered via; single-select list (see patient.drug.administrationroute)
Doage; numeric range, options: micrograms, milligrams, grams, kilograms
Since; date field
Severity; select list: severe, not severe, see serious query option

Display results by outcome (patient.drug.reaction.reactionmedrrapt).

Completed, tested, and now moved to staging.
