Cleanup:

don't extend from UnexpectedValueException
remove whitespaces

Modifications in Account:

Fetch account name from record 1.
Include the account description

Add new fields in Statement:

sequenceNumber
newDate
transactionCode

modifications for test:

upgrade phpunit to 9.5
make tests compatible with phpunit 9.5
modify sample6 to match Codabox CODA files
