What's the URL of the page that contains a bug?

Does it work before? If not, when did you find this problem?

it worked before, the problem just started today chrome gives error 
Uncaught Error: Dynamic create failed WScript.Shell

What steps will reproduce the problem?
1. when running activex object

if you enable npapi in chorme it works

chrome://flags/#enable-npapi
after enabling npapi then relaunch the chrome thats it
