Add support for AAMVA Version 4 (2009) #11

Version 4 is pretty much the same as version 7, and the parser only needed a case statement added to support these cards. I have some sample data, but it is not anonymized properly. Let me know how you would like this to be incorporated and I will amend this pull request.

Looking at the other pull request, it seems that a more-complete job of supporting AAMVA versions has already been completed. I can provide test data for version 4.
