Document the callback arguments/usage #33

The README should document how the callback() is meant to be used. It seems that the first argument is an Error. I see in winston-mongodb that a second true argument is passed, but I have no idea why.

These sorts of details are probably critical to successfully implementing a proper custom Transport.
