Sorry for bumping, but I have encountered the same issue that I believe is what OP experienced. The screen only plays the heal effect. If it helps, I think this happens when you die to an instakill. I was killed by the mimics in this example.

I see the problem. Seems like instakill don't set your player health to 0 which is what we are using to check if the player has died.
