Need a screen mockup for the Mentor Profile View

From this we don't really that much info only these fields I recon:

• First name
• Last name
• Username
• Email
• Gender
• State
• Address
• City
• Zip Code
• Phone 1
• Phone 2

We need a new option to manage some settings for Labtab:

• Sync videos when broadband connectivity is detected
