#163 - splitting code table 4.2 into its subtables #252

I splitted code table 4.2 into its subtables, by disciplines and categories. I kept the original code table 4.2 in this commit to help review what I did. I have also added the usual ranges for local usage in some tables where it was missing, for consistency
